I actually have just become aware of the importance of having a Power of Attorney Form even if I am not rich. There are different forms actually: Last will and testament, Child travel consent, Living will and health care directive. Im going to take advantage and sign up a Medical Power of Attorney form to designate someone to direct the medical care I will receive in the event that I am unable to make my own health care decisions.
